Neck Wrinkle Types: Why Horizontal and Vertical Lines Need Different Care
Horizontal lines, vertical bands and skin texture laxity are distinct types of neck wrinkles, each with different causes and different factors that shape results.
Written by ATTITUDE Clinic Editorial Team
Many people wonder whether horizontal neck wrinkles and the vertical bands that stand out when the neck tenses can be treated with the same procedure. Neck wrinkles may look like they differ only in direction, but skin thickness, muscle movement and elasticity all interact, so the approach differs. Breaking things down by which line is central also makes it clearer what needs to be decided at consultation.
The short answer first
Neck wrinkles are often grouped into horizontal lines, vertical bands, and skin texture or laxity. Horizontal lines center on the direction and depth of skin folding and skin thickness, vertical bands on pull from the neck muscles, and texture or laxity on reduced elasticity. So even when the same lifting device is used, energy intensity and depth, any accompanying injectables and the intervals are set differently for each person. The reason to sort neck wrinkles into types is not to pick a procedure name but to decide which layer needs to be addressed.
How do horizontal and vertical neck wrinkles differ?
Lines that fold horizontally when you bend your neck cross the front of the neck. These lines involve the habit of folding the skin, skin thickness and elasticity together. In contrast, the vertical bands that rise when you stretch your neck long or tense it relate to the downward pull of the neck muscles. When the direction of the line differs, the layers looked at in consultation differ too.
When horizontal lines persist, the line itself deepens and the surrounding skin texture roughens. Here, a skin booster or lifting that addresses skin texture, hydration and elasticity may be considered first. Vertical bands often become more prominent when expression or posture changes, so whether treatment to reduce muscle movement is appropriate is reviewed as well. Even within the same neck wrinkles, muscle and skin texture are intertwined, so no single method settles everything.
In the texture and laxity type, fine surface wrinkles and a sagging contour stand out more than lines. Skin thickness and regeneration speed vary from person to person, so the same energy intensity is received differently. The ATTITUDE Clinic column also cites skin type, skin thickness and structure, and regeneration speed as reasons results differ for the same procedure. ATTITUDE Clinic column
If you have had blisters or pigment changes before, you must mention this before treatment. Responses to the same stimulation can differ, and this becomes the basis for adjusting intensity and range at consultation.
Why do results differ for the same neck wrinkle treatment?
The first reason results diverge even with the same procedure name is the starting condition. How thin the skin is, how much collagen-producing capacity remains, and which treatments you have had before all affect how the same stimulation is received. The ATTITUDE Clinic column explains that skin type, thickness and regeneration speed differ between people, so the same lifting and laser treatments feel different too. ATTITUDE Clinic column
The second is the plan. Even within one lifting device, HIFU (a method that focuses ultrasound to deliver heat to deeper skin layers) and monopolar radiofrequency (a method that uses current to heat tissue beneath the skin) differ in the layers they stimulate and in how they feel. According to American Academy of Dermatology materials, radiofrequency is described as heating tissue beneath the skin, and skin firming procedures offer many options, so it is important for a clinician who knows your condition well to choose a suitable method. AAD
The third is how the treatment is carried out. Even with the same planned shot count and intensity, whether intensity is adjusted based on skin response during the procedure and how anesthesia is managed change how it feels and how you recover. ATTITUDE Clinic keeps to a workflow in which the doctor performs the procedure directly, so the plan set at consultation and the actual treatment do not drift apart. ATTITUDE Clinic column
Neck wrinkle consultation starts with deciding which layer of my neck is the issue, not with a device name. When this order is reversed, it is easy for expectations and results to diverge even after a procedure with the same name.
What criteria should guide the choice of neck wrinkle treatment?
Neck wrinkle treatment can broadly be divided into injections and skin boosters that fill skin texture and hydration, energy devices that heat beneath the skin, botulinum toxin that reduces muscle movement, and dermal filler that adds volume. The ATTITUDE Clinic treatment and price guide lists Ultherapy (HIFU lifting), Thermage FLX (monopolar radiofrequency lifting), Shurink Universe (HIFU), Onda, Potenza (RF microneedling), skin boosters, and an add-on for filler in the neck wrinkle area. What separates these is not the device name but the depth the stimulation reaches and the recovery period the skin can handle. If the horizontal line is deep and the skin is thin, filling texture and hydration first may be better than strong energy. Conversely, if laxity is central, lifting that heats the layers beneath the skin is considered first. If vertical bands are pronounced, whether treatment to reduce muscle movement is appropriate is checked.
Neck wrinkle filler is sometimes consulted for the purpose of filling the line itself. However, filler duration and side effects vary with the product, area and volume, so how long the effect lasts is decided at consultation by reviewing the product and skin condition together. In summary For neck wrinkles, rather than choosing treatment by line direction alone, skin thickness, muscle movement and elasticity should be reviewed together. Horizontal lines and vertical bands involve different layers, so even when planned on the same day, the order and intensity are set separately.
How are the intervals and number of neck wrinkle sessions decided?
The intervals and number of neck wrinkle sessions vary with the device, product and skin response. The ATTITUDE Clinic column explains that recovery speed, how much downtime you can accommodate, and how treatments affect each other are reviewed together to set the order and intervals. ATTITUDE Clinic column
When lifting and skin boosters are planned around the same time, the stimulation the skin receives overlaps. Whether to proceed at once or to split them after watching recovery depends on how much redness and swelling appear and on your schedule. Rather than presenting it as if there were a common interval fixed in numbers, it is set at consultation by reviewing previous treatment records and recovery speed.
When effects appear also depends on skin regeneration speed. The rate at which collagen is newly produced varies with age, lifestyle habits and hormonal status, so even the same procedure is felt at different times. ATTITUDE Clinic column
If this applies to you
If a wedding or photo shoot is a few weeks away
Procedures that leave redness and swelling are planned with an interval from your schedule. Rather than choosing a method with a short recovery period, first calculate how much time the skin has to settle before that day. Telling the clinic the treatment date and any fixed commitments at consultation helps adjust intensity and range.
If you want ongoing care
Neck wrinkles often do not end with a single session. It is realistic to decide whether horizontal lines, vertical bands or laxity come first, and to set the next step after recovery is complete. The clinic may advise continuing moisturizer and sunscreen down to the neck.
If your skin is sensitive and reddens easily
Even with the same energy, redness and tenderness may linger longer. Before treatment, tell the clinic about recent skin reactions, the cosmetics you use and any medications you take, and split the steps while watching your response rather than concentrating strong stimulation all at once.
If you are an international visitor on a short stay
First calculate whether the recovery period fits within your departure schedule. If there are not enough days to check progress after treatment, choose lower intensity or a method with shorter recovery at consultation. Sharing previous treatment records, medications and any allergic reactions makes planning easier.
Please tell us before consultation Let us know about recent sun exposure and outdoor plans, medications you are taking and active ingredients you apply to the skin, records of previous lifting, filler or botulinum toxin treatments and how you responded, and dates such as a wedding or shoot when your skin needs to have recovered. Telling us about pregnancy or breastfeeding, allergies, and whether you are being treated for a skin condition also helps us reflect it in the plan.
How ATTITUDE Clinic proceeds
Precise diagnosis and skin analysis Skin condition is recorded with a device, and the direction and depth of neck wrinkles, skin thickness and elasticity are reviewed together. The analysis tool only records and organizes information and does not make a medical diagnosis; judgment is made by the doctor. ATTITUDE Clinic column
Doctor consultation Your concerns, lifestyle habits and previous treatment experience are added to the records to decide what is needed and what is not, and which combination and order to use. Choosing an add-on for filler in the neck wrinkle area or selecting a lifting device is also discussed at this stage. ATTITUDE Clinic column
Treatment performed by the doctor The doctor performs the procedure directly according to the plan set at consultation, adjusting intensity while watching skin response during treatment. ATTITUDE Clinic column
Recovery and care Redness, swelling and tenderness are checked after treatment, and the next interval is set according to recovery speed. ATTITUDE Clinic column

“I take injection pain seriously.”
Co-Director Dr. Lee Dong Hun
Many people hesitate over injection treatments because they hurt. So we went back over everything, from the anesthetic to the products used.
Careful numbing, unhurried timingWe allow plenty of time for a special numbing cream to take full effect, and add anesthetic injections for the treatments and areas that need them.
Fine needles, finished quicklyInjection treatments use fine needles, and the doctor finishes them personally in a short time, adjusting along the way by watching how you are doing.
Even the product formulation countsThe same treatment can feel more or less demanding depending on the product, so we take that into account too.
The anesthetic method and whether it is used are decided after an examination, according to the treatment, the area and your condition. How much it hurts varies from person to person.
Lifting devices discussed for neck wrinkle consultation include Ultherapy (HIFU), Thermage FLX (monopolar radiofrequency), Shurink Universe (HIFU) and Onda, and injectable options include skin boosters and an add-on for filler in the neck wrinkle area. ATTITUDE Clinic treatment and price guide Devices and products are chosen differently depending on skin condition and concerns, and prices in the treatment and price guide are listed as standard prices per unit with whether VAT is included noted separately.
Aftercare and side effects
Soothing After treatment, start with the soothing stage while checking skin response. While redness and heat remain, avoid strong cleansing and exercise, and follow the care instructions given by the clinic.
Moisturizing Dryness can make itching or fine wrinkles more noticeable, so keep up moisturizing. Right after treatment, it is safer to use products recommended by the clinic.
Sun protection While the skin is sensitive after treatment, sun exposure can raise the risk of pigment changes. Apply sunscreen down to the neck when going out.
Checking progress Check whether redness, swelling and tenderness are decreasing, and set the next treatment interval after reviewing recovery.
Common reactions are redness, swelling, tenderness when pressed, and bruising. They usually settle within a few days. Rarely, temporary pigment changes, blisters, lumps after injection, burns or scars can occur. ATTITUDE Clinic column
If pain that does not improve and keeps worsening, or skin changes different from what you expected, contact the clinic without waiting. If swelling or bruising is heavily concentrated on one side during recovery, or blisters form, check these by the same standard.
Frequently asked questions
Can horizontal and vertical neck wrinkles be improved with the same procedure?
When both are present, the layers are planned separately rather than handled only with the same device at once. Horizontal lines are reviewed first for skin texture and thickness, and vertical bands for muscle movement. Even when treated on the same day, intensity and range may differ, or the order may be split after watching recovery. It is difficult to state that one procedure resolves everything.
How long do neck wrinkle filler effects last?
It depends on the product and volume, the depth of the neck wrinkles and how the skin accepts the product. Even with the same product, the neck moves frequently, so duration may feel different from person to person. At consultation, the clinic reviews which product, how many cc, which layer to place it in, and any previous filler history. Rather than promising a duration as a number in advance, the timing of touch-ups is set by watching progress.
Can neck wrinkles be improved with Botox?
If vertical bands stand out when you tense your neck, botulinum toxin to reduce muscle movement can be considered at consultation. Conversely, if deep horizontal folds, skin texture or laxity are central, Botox alone may not be enough. Muscles around the neck are involved in swallowing and vocalization, so dose and injection points are set carefully. Before treatment, the movement of the neck muscles and skin thickness are checked.
How much downtime is there after neck wrinkle treatment?
It depends on the device, energy intensity and treatment area. Redness, swelling and tenderness may remain for a few days, and injectable treatments may cause bruising or lumps. The American Academy of Dermatology advises that recovery periods differ by procedure and that you should ask your clinician when you can return to daily activities. AAD If you have fixed commitments, tell the clinic the dates before treatment so intensity can be adjusted.
How should I choose a clinic for neck wrinkle treatment?
What is checked at consultation matters more than the device name. The American Academy of Dermatology advises asking about goals, recovery period, risks and costs before a procedure, and checking the clinician's experience and how many patients with similar skin types and age groups they have treated. AAD In Korea, you can also look at whether the same doctor handles both consultation and treatment, which devices and products are used, and whether previous treatment records are reviewed. ATTITUDE Clinic column Whether prices are disclosed per unit and whether VAT is included is another point to check.
